The Episcopal Health Foundation awards Integral Care $400,00 to continue crucial partnerships which expand Primary Care Services to Integral Care clients and increase Integral Care’s operating capacity through support of Community Health Workers. Through last year’s funding, Integral Care has contracted with a Federally Qualified Health Center that now provides onsite primary care services at two of our outpatient behavioral health clinics. This year’s funding aims to scale EHF investments over three years to increase access to primary care and offer Community Health to engage underserved communities. Integral Care provides counseling and behavioral health services to the population and connects individuals with integrated primary care. This project is a unique opportunity to expand our service offering beyond mental health and offers primary care on-site, virtually, and in person. Integral Care has contracted with a local Federally Qualified Health Clinic (FQHC) to provide on-site primary care services for general Integral Care clients. Integral Care has also hired Community Health Workers to serve individuals at home, in the community, or virtually. The primary goal of this work is to provide an innovative form of care that addresses all healthcare needs where it is convenient for the client, whether it is virtually, on-site at our mental health clinic, or in the community.
