Building Health and Stability Together: $3,006,147 Texas Health and Human Services Award Strengthens the Healthy Community Collaborative

Integral Care is honored to receive a $3,006,147 award through the Texas Department of Health and Human Services Healthy Community Collaborative (HCC) initiative, supporting a comprehensive, multi-year effort to serve individuals experiencing homelessness across Travis County. This project will focus on people living unsheltered and those in permanent supportive housing who are also facing co-occurring mental health and substance use disorders. By integrating outreach, behavioral health care, and supportive services, the program aims to improve stability, health outcomes, and long-term recovery. If awarded, the initiative may be renewed for up to five years, allowing for sustained engagement and deeper impact over time.
